🏥 Hospital Death vs. Home Death: The Process

The application method depends strictly on where the death occurred. Following the wrong process will lead to immediate rejection.

Case 1: Institutional Death (Hospital)

If the death happened in a Hospital/Health Center:

❌ DO NOT Apply Online: The hospital authorities will automatically send the data to the Municipality/BDO.
✅ What to do: Collect the “Medical Certificate of Cause of Death” from the doctor and wait 21 days to download the certificate online.

Case 2: Domiciliary Death (Home)

Login & OTP: Enter your (Applicant’s) Mobile Number. Verify the OTP to access the form.

Enter Deceased Details: Fill in the Date of Death, Gender, and Full Name exactly as per their Aadhar Card. Select “Place of Death” as Home.
